Public Domain Templates for ArcPad
ArcPad lends itself to a variety of industry applications. The ability to collect information in any location and store it in a spatial database enables improved processes and new efficiencies. The following templates highlight specific applications of ArcPad technology to solve industry problems. These templates vary from tools to applets to forms, all of which illustrate the practical benefits of ArcPad. These templates aren't designed as end solutions; rather they are a starting point for creating your own ArcPad project.
The ArcPad templates are located on ArcScripts, where you can search and download various scripts and sample codes contributed by ESRI's user community.
Health and Human Services
Emergency Medical Services—Accident Data Collection Form
This script replicates the basic "ambulance run" form that is used to collect patient information on arrival and during transport. The form is based on a Scantron F-1612-ICEMA, which is used by most emergency medical technicians. Download the Accident Data Collection applet.
Mosquito Control Field Inspection Template
This template replaces paper-based inspections of mosquito incidence and collects GPS coordinates for sites. This template includes electronic forms for recording new site locations and collecting information about adult and larvae mosquitoes, types of habitats, and treatments if any were implemented. Mosquito Control Field Survey
This template allows users of ArcPad to collect information about the location and habitat of mosquitoes. This template collects attribute data on adult mosquitoes, larvae, habitats, and treatments that have been applied. Download the Mosquito Control survey.
Mosquito Resistance Testing Template
This template allows users of ArcPad to collect information on the testing and evaluation of insecticides. This template is designed to collect insecticide resistance data from a grid pattern of mosquito cages. Download the Mosquito Resistance Testing template.
Patient Registration
This script allows ArcPad to collect general patient registration information used by a doctor's office, health facility, or emergency room. This is a nonspatial form that accepts all personal information and updates a Microsoft Access database via a wireless local area network. Download the Patient Registration applet.
Smallpox Post-Event Surveillance Form
This script replicates the Center for Disease Control's Smallpox Post-Event Data Collection form. Data validation has been incorporated into the script along with the ability to print the form. The script can be easily modified for the direct input of longitude/latitude either manually (by reference) or by GPS. Download the Smallpox Post-Event Surveillance form.
World Health Organization Village/Dengue Surveys Form
This script enables ArcPad to be used as a custom data collection system for World Health Organization (WHO) health monitoring programs. It includes the WHO Village Mapping Survey questionnaire and WHO Dengue Fever questionnaire. This script allows the direct input of attributes and longitude/latitude into a local database as well as updating a RDBMS via wireless data transfer. Download the WHO surveys.
Infrastructure and Utility
Septic System Inspection Template
This template allows ArcPad to be used to collect information and conduct inspections on septic systems. Users can enter data through a custom data entry form. Sample data is included for parcels, waterlines, sewer lines, streets, septic, and soils. Download the Septic System Inspection template.
Storm Water Inlet Inventory Template
This template allows users to collect information about storm water inlets with ArcPad. Included is a data entry form that collects information for inlet type, grate style, ownership, physical condition, road name, and more. Download the Storm Water Inlet Inventory template.
Storm Water Management Tool
This tool helps facilitate storm water management using ArcPad. This tool includes a data collection form for recording storm water data in compliance with the National Pollution Discharge Elimination System guidelines. Download the Storm Water Management tool.
Storm Water Outfall Template
This template is designed to assist county or state highway agencies comply with storm water regulations and manage the location of outfalls within or nearby the road right-of-ways. Information, such as pipe diameter, system type, material type, and receiving water, can be collected. This application also allows the user to collect basic inspection information such as erosion, scour protection, and general condition. Users can identify the location of the outfall by route number and mile marker or with a GPS. Download the Storm Water Outfall template.

Agriculture and Natural Resource Tools
This suite of tools allows agriculture and other natural resource field-workers to make sampling grids, calculate statistics for agricultural attributes, select which layers should be editable, and navigate to specific field locations using ArcPad. Download the ArcPad Agriculture template.
Beaver Nuisance Survey Template
This template allows users to accurately and efficiently record the position of nuisance beaver structures. With this template, nuisance structures can be logged using ArcPad and scheduled for removal. This template aids work flow and data entry by autopopulating the data collection layer with attribute information from other shapefiles. Download the Beaver Nuisance Survey template.
Public Safety
Aerial Survey—Data Collection Applet
This ArcPad applet was designed for aerial survey data collection (data collection from a helicopter or airplane). This applet facilitates data collection in less than optimal conditions such as high vibration and rapid movement. Data collection is handled by a special modified form with large buttons requiring minimal user input. Download the Aerial Survey applet.
Fire Perimeter Mapping
The Mobile Fire Mapper applet provides tools to map a fire perimeter using ArcPad. The perimeter can be recorded using a GPS, stylus, or mouse. Download the Mobile Fire Mapper applet.
First Responder Accident Data Collection Form
This form collects accident data and road conditions for reporting and analysis using ArcPad. This form has data entry fields for date, accident type, milepost, alcohol related, hit and run, road type, and weather condition. Download the First Responder Accident Data Collection form.
Rural Fire Fighting Resource Inventory
This template allows ArcPad to be used to support rural fire fighting and mitigation. This template is used to create an inventory of drafting sources and dry hydrants often found in rural settings. This template identifies resources that emergency responders can access when fighting fires. This template includes custom toolbars and forms to ease data entry with validation rules, to assure correct data types and logical responses from users. Structural Fire Risk Assessment Template
This template allows firemen and other field-workers to assess risk on every building within a county. The template's data collection form increases the data quality of the master address database by requiring street names and numbers for each building. Environmental Code Enforcement Template
This template is used with ArcPad to improve environmental code enforcement and patrol illegal dumping. Download the Environmental Code Enforcement template.
Equipment and Inventory Maintenance Template
This template demonstrates how ArcPad can be used for maintaining an inventory and tracking maintenance. This template includes forms for recording information about playground equipment, play areas, and maintenance of the equipment. Download the Equipment and Inventory Management template.
GASB34—Municipal Asset Collection Form
This template illustrates the use of ArcPad to collect information and inventory municipal assets for GASB34 financial reporting standards. This template includes a toolbar that launches customized forms to collect information for a variety of municipal assets. Download the Municipal Asset Collection form.
Mobile Public Inquiry Tool
This tool allows city officials to use ArcPad to record public inquiries and complaints while marking their locations on a map for follow-up. This tool creates a mobile interface to an ArcIMS mapping server and Oracle database. Download the Mobile Public Inquiry tool.
Public Nuisance Survey—Data Collection Form
This applet file allows ArcPad to collect information about the location and
description of various types of public nuisances. It provides a customized data
collection form that is loaded with the sample data file. Layer definitions are
stored in a file associated with the shapefile. Download the Public Nuisance Survey form.
Rural Code Enforcement Template
This template is designed to facilitate enforcing rural code compliance and reporting with ArcPad. Users are able to locate the parcel in violation, see ownership information, and record the type of violation for reporting purposes. This template collects attributes such as assessor's parcel number, ownership information, type of violation (by dropdown list), date, and general comments. Housing Condition Survey—Data Collection Form
This ArcPad form allows municipal planning and community development agencies
to collect housing condition information using a mobile housing condition survey.
Housing condition data collected in the field is entered once and can be uploaded
to the main GIS database for immediate use, thus reducing the second
step of data entry in the office. The information allows
communities to assess housing stock conditions and is an essential part
of a community's housing development strategy. Download the

Mobile Zoning Survey
This form allows users to collect information about areas in a city, to use in comprehensive zoning. Download the Mobile Zoning survey.
Neighborhood Tree Survey—Data Collection Form
This applet allows ArcPad to collect location and descriptive information about the trees in a community. Included is a data entry form with error checking, menus, and images of tree leaves to aid in tree identification. The information collected is useful for assessing the economic value of trees, pollution abatement potential, and ideal sites for new tree plantings. Download the Neighborhood Tree Survey form.
Parks Inventory Form
This template allows the user to conduct an inventory of parks, including playground equipment, athletic fields, trees, and any other amenities within a park. This inventory is then used to facilitate an in-depth analysis of a municipality's park system. Attributes collected include: park name, object, quantity, comments, and date. Tree Inventory Template
This template allows users to create an inventory of a municipality's trees using ArcPad. This template supports data collection with or without a GPS. Attribute information is also collected to facilitate the planning of maintenance. Download the Tree Inventory template.
